At DeKalb Memorial Hospital, we believe in planning ahead – that’s why we encourage all of our patients to register in advance. Online Registration is a convenience for patients and the hospital alike. It is a way we can ensure important paperwork is prepared before a patient’s anticipated arrival, saving valuable waiting time and allowing staff to expedite the check-in process.

Important to note:

Please submit your online registration form 24 hours before a test or procedure. If your test or procedure will take place sooner than 48 hours, please call the Admission Department at 260.920.2603.
Upon arrival, proceed directly to the department performing your test or procedure. Any other necessary forms will be completed at this point. EXCEPTIONS: LABOR & DELIVERY PROCEDURES MUST STOP BY THE ADMISSION DEPARTMENT TO SIGN AND PICK UP THE PAPERWORK.
